Clinical Engagement and Partnerships CoordinatorThis is a part time, permanent position, Monday, Wednesday and Friday, 9am to 5pm, based at our Concord office with travel across Sydney.
You'll own the front door — building and holding the relationships that bring participants to us, and making sure every referral that arrives is handled properly and quickly.Key responsibilitiesBuild and maintain referral relationships with hospitals, discharge planners, clinical teams, allied health and community organisationsManage referrals as they come in — triage, respond, gather what's needed and hand over cleanly to the service teamRepresent Aus Care Community Services at meetings, network events and with referring professionalsTrack where referrals come from and what happens to them, and report on itWork with the Managing Director on which relationships and areas to prioritiseMake sure referral conversations reflect participant choice and our conflict of interest obligationsAbout youA background in disability, health, mental health or community services, with credibility in front of clinical professionalsExperience in referral management, intake, partnerships or business development in a health or disability settingSolid working knowledge of the NDIS,



particularly how participants come into the scheme and what happens at dischargeConfidence starting conversations with people you don't know yetOrganised follow-up — the relationships are built in the second and third contact, not the firstCurrent NDIS Worker Screening Check, Working with Children Check, driver's licence and comprehensively insured vehicleAbout usAus Care Community Services is an NDIS Quality and Safeguards Commission registered provider based in Concord, supporting participants across Sydney and NSW.
