This part time role focuses on delivering person-centred home care to clients in Geelong, Ballarat, and surrounding areas, under RN oversight.Primarily office-based with community visits, you will perform assessments, care planning and coordination, maintaining high standards of safety, ethics and confidentiality in line with aged care regulations.
