Job Description
Western Health is seeking an experienced Case Manager (Clinical Coordinator/Allied Health Professional) for the Bed Based Transition Care Program at Williamstown Hospital. This part-time role coordinates complex care needs and discharge planning, guiding older adults and families through community and residential care pathways.
