Job Description
Latrobe Regional Health is seeking a HIP Care Coordinator for a part time ongoing role (0.5 EFT) in Gippsland. The role involves assessing, coordinating and managing a caseload of Health Independence Program clients to support better health outcomes and independence.

Ideal candidates will be an experienced Division 1 Registered Nurse with AHPRA, hold a Victorian Driver Licence, and have at least five years in acute care.
