Calvary Riverina Hospital in Wagga Wagga, NSW is seeking a compassionate Social Worker to provide psychosocial assessment, support and advocacy across the hospital, with a focus on rehabilitation, palliative care and complex health journeys.This permanent, part time role involves discharge planning, care coordination and working with a multidisciplinary team to support patients and families through illness, recovery and end-of-life care.
Monday to Friday, business hours.
