Primary Care Connect is seeking a Registered Nurse to join our Primary Health Team in Shepparton on a part time fixed-term basis until 30 June ****.
The role focuses on chronic disease education and early intervention, delivering direct care to clients with COPD, cardiovascular disease and diabetes, and assisting with self-management strategies.
You will work with the broader PCC team, providing range of primary care initiatives and direct client services, in line with best practice standards
