Job Description

Wellways Australia seeks an Intake Worker for Connected Care in Northern Tasmania. You will be the first contact for participants, supporting intake, screening and assessment and using IAR-DST to tailor care plans for timely access to services.

You will collaborate with peer workers, clinicians and partner organisations in a multidisciplinary team, promoting informed choice, autonomy and self-determination while maintaining safety and escalation protocols.
