About Forming Connections

Forming Connections is a neuro-affirming, trauma-informed NDIS provider specialising in supporting autistic, ADHD, and other neurodivergent children, young people, and adults.

We are passionate about providing individualised, strengths-based support that celebrates neurodiversity, respects autonomy, and improves quality of life. Our approach moves beyond traditional behaviour management, focusing instead on understanding individual experiences, recognising unmet needs, supporting emotional and sensory regulation, and creating environments where people can thrive.

We are committed to building meaningful relationships, strengthening the systems around people, and supporting practitioners to do their best work – without burning out.

Our team values flexibility, ethical practice, reflective supervision, and a genuine commitment to neurodiversity-affirming Positive Behaviour Support.

About the Opportunity

We are seeking a compassionate, person-centred, and clinically minded Behaviour Support Practitioner to join our growing Queensland team.

This opportunity is particularly suited to practitioners who have an interest in, or experience working with, autistic people, people with ADHD, and individuals with other neurodevelopmental differences, including those with complex support needs, co-occurring intellectual disability, and mental health challenges.

You will work collaboratively with children, adults, families, carers, schools, Support Coordinators, and other professionals to understand behaviours within the context of each person’s neurotype, communication, sensory experiences, relationships, and environment.

We recognise that behaviours of concern often reflect unmet needs, distress, communication differences, or difficulty navigating environments not designed to accommodate neurodivergent ways of experiencing the world.

Our practitioners are encouraged to look beyond behaviour itself and focus on connection, felt safety, autonomy, emotional regulation, and meaningful participation in everyday life.

About the Role

- Conduct comprehensive functional behaviour assessments that explore the underlying functions of behaviours, communication differences, sensory needs, environmental factors, and unmet support needs.
- Develop and implement evidence-based, rights-focused, neuro-affirming Positive Behaviour Support Plans tailored to each person’s needs, strengths, goals, and circumstances.
- Support autistic, ADHD, and other neurodivergent children and adults to develop emotional regulation, communication, self-advocacy, and everyday living skills.
- Work collaboratively with families, carers, educators, and support teams to develop practical strategies that promote safety, connection,

predictability, and autonomy.
- Provide education, coaching, and training to families, schools, disability support workers, and other stakeholders.
- Apply trauma-informed, low-arousal, and collaborative approaches that prioritise understanding distress over compliance-based behaviour management.
- Identify opportunities to reduce and eliminate restrictive practices through proactive environmental adjustments, skills development, and capacity building.
- Monitor outcomes and review support plans to ensure strategies remain meaningful, effective, and responsive to the person’s changing needs.
- Collaborate with psychologists, occupational therapists, speech pathologists, educators, and other professionals to provide coordinated support.
- Maintain high-quality clinical documentation and reports consistent with NDIS requirements and professional standards.
- Participate in regular reflective clinical supervision, professional development, and team collaboration.
- Work autonomously from home and in the community while remaining connected to an experienced and supportive clinical team.

Who We’re Looking For

We welcome applications from practitioners who share our commitment to neurodiversity-affirming and rights-based practice.

Essential or highly desirable attributes:

- Recognition as a Suitable Behaviour Support Practitioner at Core, Proficient, or Advanced level is highly desirable.
- Relevant qualifications in Psychology, Social Work, Occupational Therapy, Speech Pathology, Education, Counselling, or a related discipline.
- Experience supporting autistic people, people with ADHD, intellectual disability, psychosocial disability, or complex support needs.
- An understanding of neurodiversity-affirming practice and the importance of respecting individual differences in communication, sensory processing, regulation, and social interaction.
- Ability to develop clear, practical, respectful, and high-quality behaviour support plans and reports.
- Commitment to trauma-informed, person-centred, and evidence-based practice.
- Strong communication and collaborative skills when working with children, adults, families, and multidisciplinary teams.
- Ability to work independently, manage a flexible caseload, and maintain accurate clinical records.
- Willingness to engage in reflective supervision, ongoing professional learning, and collaborative clinical discussions.

We would particularly love to hear from practitioners who:

- Have experience working with neurodivergent children, adolescents,

and/or adults.
- Are passionate about supporting people with complex presentations, including co-occurring autism, ADHD, intellectual disability, anxiety, trauma, and mental health conditions.
- Recognise that behaviour is communication and that meaningful change often begins with adapting environments and strengthening relationships.
- Value collaborative problem-solving, autonomy, sensory-informed approaches, and reducing demands that contribute to distress.
- Want to practise Positive Behaviour Support in a way that genuinely respects neurodivergent identity and lived experience.
