- Monitor changing needs and risks, escalating clinical concerns appropriately.
- Coordinate complex supports, including restorative care, end-of-life care, and assistive technology/home modifications.
- Work closely with participants, families, clinicians, and care teams to achieve positive outcomes.
- Support reassessments, case conferences, and ongoing care planning.
- Maintain accurate, timely documentation and meet quality and compliance requirements.
- Promote dignity, informed choice, supported decision-making and culturally safe care.

- Relevant medication support and healthy body systems competencies.
- Current driver’s licence and access to a comprehensively insured vehicle.
- Current National Police Certificate, or ability to obtain.

Skills & Experience
- Experience in care coordination, case management, home care, aged care, health, or disability services.
- Sound understanding of Support at Home, CHSP, aged care quality standards and participant rights.
- Strong care planning, coordination, and risk-management skills.
- Ability to recognise changing or complex needs and escalate concerns appropriately.
- Excellent communication, relationship-building and problem-solving skills.
- A strong person-centred, culturally secure, and reablement-focused approach.
- Confidence managing participant budgets and coordinating services.
- Competence using digital care management systems and Microsoft Office.
- Experience with My Aged Care, dementia, complex ageing, or palliative care is highly regarded.
Why join us?
- Up to $18,549 tax-free salary packaging
- Generous leave entitlements (5 weeks annual leave)
- A supportive and positive workplace culture
- Meaningful work where you can make a genuine difference in people’s lives
- Opportunities for ongoing learning, development, and professional growth
- Be part of a values-driven organisation committed to quality, dignity, and compassionate care
If you’re passionate about empowering older Australians to live independently and want to be part of a team that genuinely cares, we’d love to hear from you
