Our services include Specialist Homelessness Services, aged care, disability, settlement and other community programs. We are committed to providing practical, culturally responsive and person-centred support that promotes safety, independence and positive outcomes.
We are looking for a Case Worker to join our SHS team and support people who are experiencing or at risk of homelessness.
About the Role
This is a hands-on case management role focused on helping clients navigate homelessness, housing and other complex life circumstances.
You will work closely with clients to understand their needs, develop practical support plans, advocate on their behalf and connect them with appropriate housing and community services.
The role will involve working with housing providers, government agencies, SHS and community organisations, as well as providing outreach and practical support to clients in the community.
You will also have an opportunity to help strengthen ADSI's local housing partnerships and referral networks.
Key Responsibilities
- Conduct client assessments, intake and holistic case management.
- Develop and review individualised case plans with clients.
- Provide advocacy, referrals and practical support to help clients secure and maintain appropriate housing.
- Assist clients to navigate social, community and affordable housing options.
- Identify and refer clients to appropriate crisis, emergency and temporary accommodation services.
- Support clients to access services and attend appointments, including Centrelink, housing and other government services.
- Work with clients experiencing complex circumstances, including homelessness, domestic and family violence, mental health concerns, substance use and/or involvement with the criminal justice system.
- Maintain accurate case notes, records, referrals and reporting in accordance with program and organisational requirements.
- Work collaboratively with relevant government, housing, health, DFV and community services.
- Contribute to the development of ADSI's SHS program, referral pathways and community partnerships.
What We're Looking For
We are looking for someone who is caring, practical and motivated to make a genuine difference in people's lives. The successful candidate will ideally bring:
- Bachelor's degree in Social Work – highly preferred. Relevant professional registration or eligibility for registration with the Australian Association of Social Workers (AASW) is desirable.
- Case work experience in Specialist Homelessness Services, housing or the broader community services sector is highly desirable, although we are open to candidates with transferable experience and a strong willingness to learn.
- Strong understanding of holistic case management, social work principles and professional standards.
- Experience or demonstrated ability to work with complex and high-risk clients, including people experiencing homelessness, DFV, mental health concerns,
substance use and/or criminal justice involvement.
- Good knowledge of social housing requirements, eligibility and application processes, as well as community and affordable housing options.
- Ability to identify appropriate housing and accommodation pathways and make timely referrals to crisis, temporary and specialist services.
- Demonstrated ability to work respectfully with culturally and linguistically diverse (CALD) communities, with an understanding of culturally responsive and trauma-informed practice.
- Strong communication, advocacy and relationship-building skills, with the ability to work effectively with clients, families and service providers.
- A commitment to client confidentiality, qualified boundaries, informed consent, risk management and organisational policies and procedures.
- Ability to work independently while also being a positive and supportive member of the wider ADSI team.
- A positive, practical and can-do attitude, with a genuine willingness to learn and develop.
- As a condition of employment, you will be asked to provide 100 points of ID, national police check and Working With Children’s Check
