Home With Home Homecare is looking for a Grade 1 or 2 Physiotherapist and Occupational therapist to provide in-home care to clients across Melbourne. We are a home care service provider who provide in-home supports to Support at home and NDIS clients and are expanding to providing allied health services due to demand. You'll work with private and Medicare referred clients, Home Care Package/Support at Home and NDIS, WorkSafe/TAC participants.

Community visits are primarily across North, West and East suburbs of Melbourne

Pay: $60-70 per Hour

Status: Casual or Subcontract

Key Responsibilities:

- Provide in-home Physiotherapy and Occupational therapist care to clients
- Perform Physiotherapy and Occupational therapy assessments for clients
- Provide hands-on treatment,
- Equipment and exercise prescription
- Report writing and assisting with care planning
- Develop personalized treatment plans for clients
- Monitor progress and adjust treatments as needed
- Manage your own schedule and arrange appointments with clients
- Conduct clinical handover with the Care Manager
- Maintain clear clinical records and documentation
- Communicate effectively with clients, families,



referrers, support coordinators, case managers and other health professionals
- Work collaboratively with other healthcare professionals

About Your Role:

- Strength, mobility, balance and falls prevention
- Post-operative and general rehabilitation and treatment
- Neurological and disability-related rehabilitation and treatment
- Contribute to quality improvement activities

Essential Requirements:

- Current AHPRA registration with no restrictions
- Reliable Car
- Community Physiotherapist and Occupational therapist experience
- Good communication and interpersonal skills
- Ability to work independently and as part of a team
- Valid Police check and NDIS check
- Valid First Aid and CPR certificate
- Valid manual handling certificate
