CatholicCare Sydney is seeking a Care Coordinator to deliver early intervention supports for children aged 5–11 and their families. You’ll work as part of a multidisciplinary team in Sydney’s Inner West, with flexible, hybrid arrangements and opportunities for supervision, training and career development.

You will assess needs, develop family-centred plans and connect families with health, education and community services. A strong focus on engagement and team-oriented practice is essential.
