Care Partner – Support at Home (Case Manager)

Just for u Care is an established, government-approved provider of in-home aged care and NDIS support, based in Cheltenham. We’re big enough to offer stability, and small enough that decisions happen quickly. You’ll work directly with the directors, your ideas will be heard, and you won’t be stuck waiting on layers of approval. Your experience will shape how we do things.

About the role

As a Care Partner, you'll be the main point of contact and case manager for a portfolio of older Australians on Support at Home packages. You'll manage the full client journey, from assessment and commencement through care planning, budgets and ongoing review, so clients can stay safe and independent at home. You'll be part of a close team of Care Partners from nursing and allied health backgrounds, supported by a Senior Care Partner and our operations, rostering and compliance teams.

Key responsibilities

Manage a caseload of Support at Home clients from assessment through to ongoing care

Develop and review person-centred care plans and manage client budgets

Coordinate services with support workers, GPs,



allied health and community providers

Keep regular contact with clients and families, including monthly check-ins and home visits

Keep accurate case notes and meet the Aged Care Quality Standards

About you

Case management experience in home care, community or aged care

Support at Home or Home Care Package experience is highly regarded

A nursing qualification (EN/RN) or allied health background is preferred

Strong organisation and confidence managing a full caseload

Warm and clear with clients, families and other professionals

Current police check, driver's licence and reliable car

A second language is a bonus

$85,000 – $95,000 + super, depending on experience

Performance bonus paid half-yearly

Hybrid work: 4 days in office, 1 day at home

Travel reimbursed for client visits
