VHCC seeks a Community Worker in Box Hill, VIC to support NDIS participants and their families. The role focuses on assessing needs, developing individualised plans, and connecting clients with funded supports within a multidisciplinary team.

The successful candidate will work in a newly created full time position, coordinating services to help participants achieve independent living and goal attainment in the community.
