Warrigal Home Services Bundanoon is seeking an experienced Care Coordination qualified to support older individuals in their own homes. You will work with the Care Services Manager, Operations Partner and Home Services team to ensure services align with care plans and approved frameworks.
In this hybrid role, you will manage multiple client caseloads, develop and review care plans, and advocate for client needs while collaborating with families, service providers and internal teams.
