You'll look after your own clients from onboarding through to ongoing review. You'll be the main point of contact for your own group of clients, getting to know them and their families, planning care around what matters to them, and helping them stay safe and independent at home.

Key responsibilities

Welcoming new clients and building care plans around their goals, preferences and wellbeing

Reviewing care plans regularly and when needs change

Keeping in regular contact with clients and families, and being the person they call

Helping clients understand their Support at Home funding, budgets, statements and choices

Coordinating services with our scheduling team, support workers and external providers

Arranging assistive technology, home modifications, allied health, and restorative and end-of-life supports

Spotting changes in health, risk or circumstances, and escalating clinical concerns to our clinical team

Working with our clinical team on reassessments and case conferences

Keeping accurate, timely notes in our client management system and meeting Aged Care Quality Standards

Providing hands-on care when needed to support clients and the team

About you

Experience in care coordination, case management, nursing, allied health, home care or aged care

Strong care planning, organisation and time management skills

A working understanding of Support at Home,



the Aged Care Quality Standards and client rights under the new Aged Care Act

Good judgement in recognising risk and complex needs, and knowing when to upscale

Clear, warm communication with clients, families, support workers and health professionals

Confidence with client budgets, digital care management systems and Microsoft 365

Preferred: Registered Nurse or Enrolled Nurse (current AHPRA registration), or an allied health qualification

Minimum: Certificate III in Individual Support or equivalent, with robust care coordination experience

Current First Aid and CPR

Current driver's licence and a comprehensively insured vehicle
