South Melbourne Care Services is seeking an experienced, compassionate and reliable Disability Support Worker to join our team, supporting participants in Fitzroy and Coburg.
We are looking for someone who is passionate about providing person-centred support and helping participants maintain their independence, dignity and quality of life.
Key Responsibilities
- Provide personal care and daily living assistance as required.
- Support participants with community access and social activities.
- Follow individual support plans and behaviour support strategies.
- Assist with daily routines and promote independence.
- Maintain accurate progress notes and shift documentation.
- Follow all NDIS policies, procedures and workplace safety requirements.
- Report incidents and changes in participants’ needs promptly.
Essential Requirements
- Previous experience as a Disability Support Worker is essential.
- Current NDIS Worker Screening Check.
- Current First Aid and CPR certificates.
- National Police Check.
- Working With Children Check, where applicable.
- Relevant qualifications in Individual Support or Disability Support are desirable.
- All mandatory compliance documents and training must be current.
- Good communication, documentation and interpersonal skills.
- A reliable, skilled and caring approach to support work.
Location Requirements Applicants must be available to work in Fitzroy and/or Coburg
