UnitingCare Queensland – Macleay Island is seeking a dedicated Care Partner to ensure client-centred, high-quality care. You’ll oversee care planning, coordinate services, and work with clinical teams to achieve the best outcomes across the aged care service funding, while spending a valuable part of your day visiting clients in the community.

The role requires a Certificate IV in Health or Community Service (Essential), a current Australian Driver’s Licence (Essential), and a vehicle for travel.
