BlueCare, part of UnitingCare, is seeking a Clinical Care Partner to oversee and coordinate complex clinical needs for community-dwelling clients in Coomera, Pimpama and surrounding areas. The role focuses on assessment, care planning and linking clients with necessary services to achieve optimal outcomes.

You will spend most days out in the community, working with multidisciplinary teams, managing plans, budgets and care delivery, and supporting client navigation through the care system.
