Northeast Health Wangaratta is seeking a Community Care Intake Advisor to support timely response to referrals and manage intake for NHW community clinics and services.

The CCIA will work within the Community Care Intake team, applying eligibility guidelines, coordinating referrals, and contributing to efficient triage through transparent communication with medical, nursing and allied health staff. A high level of organisation and attention to detail is essential.
