About the Role

Activity Care Services is seeking female Disability Support Workers to provide personalised support to a gentleman in his 50s living in Ormeau with an acquired brain injury and Type II diabetes. The role requires experience in diabetes management, blood glucose monitoring, insulin administration, medication management, and supporting individuals with complex care needs.

We are also recruiting support workers for a female participant residing in Mount Gravatt, who is semi independent and requires assistance with in-home support, community access, and attending appointments.

We are looking for dedicated support workers who can commit to ongoing shifts and deliver high-quality, person-centred care.

Key Details

- Casual positions with potential part-time opportunities.

- Primary location: Ormeau 4208. Additional opportunities in Mount Gravatt 4122.

- Ongoing roster with consistent shifts.

- Ormeau shift times: 8:00 AM – 2:00 PM and 4:00 PM – 8:00 PM.

- Minimum 3-4 weekdays of consistent availability required.

Key Responsibilities

- Administering medication, including insulin and subcutaneous injections, in accordance with prescribed care plans and assessed competency.

- Monitoring blood glucose levels and implementing diabetes management plans.

- Supporting participants with acquired brain injuries and other disability-related needs.

- Assisting with personal care and daily living activities.

- Performing domestic duties, including cooking, cleaning, and meal preparation.

- Providing Community Access Support (CAS), including appointments, shopping, social outings, and recreational activities.

- Transporting and accompanying participants to medical appointments, allied health appointments,



and community activities.

- Encouraging independence, social participation, and community engagement.

- Monitoring participants' well-being and reporting changes or concerns.

- Working collaboratively with healthcare professionals and support teams.

- Completing accurate shift notes, medication records, and other required documentation.

- Building positive, respectful, and professional relationships with participants.

Essential Requirements

- Current NDIS Worker Screening Check.

- Current First Aid and CPR certificates.

- Current Australian Driver's Licence.

- Reliable, registered, and insured vehicle.

- National Police Check.

- Previous experience in disability support.

- Experience in diabetes management, insulin administration, or acquired brain injury support is highly desirable, particularly for the Ormeau role.

- Competence in medication administration or willingness to undertake required training and competency assessments.

- Certificate III or IV in Disability, Individual Support, or a related field (preferred).

- Strong communication and documentation skills.

- Valid Australian working rights.

- Availability for at least 3 weekdays per week, with 4 weekdays preferred.
