The role delivers outreach and home-based support, within the Housing Pathways Program. The position is based across Dawn House locations in Darwin and undertakes visits to clients' homes and community settings to meet service requirements.

Key responsibilities

- Work with women and case managers to implement and review agreed support activities, communicate progress and barriers, and contribute to case discussions and referrals.

- Help women access internal and external emergency relief and other assistance while building financial capability, and coordinate approved food, hygiene, household and donated resources as required.

- Complete client notes in SHIP before the end of shift and maintain accurate support, visit, referral and resource records.

About you

- Strong communication and relationship-building skills, including advocacy and collaboration with clients, colleagues, housing providers and community agencies.

- Accurate and timely case documentation and computer skills, including electronic client records such as SHIP.

- Hold and maintain a National Police Clearance and Working with Children Clearance (Ochre Card), and current Northern Territory Driver Licence or ability to obtain one.
