Care Coordinator — Join the Pearl team

We’re looking for a warm, empathetic and organised Care Coordinator to join our friendly, growing team.

Who you are matters to us

You’ll need experience in a similar role, but you don’t need to have worked as a Care Coordinator before. Your background might include supporting clients, coordinating services, managing rosters or helping a busy team run smoothly.

You bring your experience, kindness and willingness to learn, and we’ll help you build on them. We’ll guide you through the role, help you learn our systems and support you as you find your feet. You won’t be expected to know everything on day one.

What your day will involve

• Being a friendly, reliable point of contact for clients, families and support workers.
• Coordinating care services and rosters to help clients receive the support they need.
• Listening to clients and responding when their needs or circumstances change.
• Keeping client records and care plans accurate and up to date.
• Working with the team to resolve scheduling issues and follow up on concerns.
• Building trusting relationships through thoughtful communication and reliable follow-through.

• Robust organisation and attention to the details that matter.
• Clear communication and a calm, practical approach to solving problems.
• Confidence using computer systems and learning current processes.

Those first six months are about giving you the support, guidance and face-to-face time you need to really settle in, build relationships with the team and feel confident in the role before transitioning into a more flexible way of working.
