Polcare in Melbourne is seeking a passionate and motivated Occupational Therapist to join our growing community care team. This role supports clients across NDIS, aged care and home care services to maintain independence, safety and quality of life in their own homes and communities.

You will conduct home and safety assessments, prescribe equipment, write reports and communicate with stakeholders, delivering client-centred interventions and goal planning.
