Southern Cross Care WA is seeking a dedicated home care coordinator to manage a client portfolio and coordinate tailored services that improve wellbeing. You will act as the key link between clients, families and care teams to ensure seamless, person-centred care.

The role is predominantly work-from-home with travel to clients north of the river, requiring strong organisation, communication and compliance with funding frameworks to achieve excellent outcomes.
