We deliver Supported Independent Living (SIL), in-home care, community participation, short-term accommodation, and community nursing.About the RoleWe are seeking dedicated Support Workers to join our Supported Independent Living team in Norton Sands.
In this role, you will support participants to live as independently as possible while engaging actively in their community.Key ResponsibilitiesAssist participants with personal care and daily living activities, including meal preparation, household tasks, and medication remindersSupport participants to access the community, attend appointments, and engage in social and recreational activitiesFollow individualised Behaviour Support Plans and Risk Assessments to ensure safe, high-quality careMaintain accurate documentation, including shift notes, incident reports,
