One Dream Community is seeking a Registered Nurse or Enrolled Nurse to join our Sydney team in Burwood as an Aged Care RN Coordinator. The role focuses on coordinating care and services for older clients receiving support through the Support at Home program, including assessments and care plan development.

You will work with clients, families and health specialists to ensure timely services, clinical governance and culturally responsive care in home settings across NSW.
