Heather Senior Care is seeking a Clinical Care Partner (RN) to support seniors in their homes from our Milton base. You will navigate care plans, set goals, and provide compassionate, autonomous nursing support with a company vehicle.

You will travel across Brisbane suburbs, visiting clients, conducting reviews, and coordinating with carers to ensure wellbeing and independence for older adults in the community.
