Bayside Health is seeking an Aged Care Assessor (Nurse or Allied Health) to join our Hastings Community Health team. The role involves conducting comprehensive aged care assessments with clients and regular travel within the catchment to homes and clinics.

You will work within the SAS framework determining eligibility for CHSP, Home Care Packages, and other services. The role offers permanent full time and part time positions, with opportunities to apply clinical knowledge to improve access and
