Bayside Health is seeking an experienced HITH nurse to deliver high-quality home-based care across Melbourne's regions. You will work autonomously in the community, assess patients, and coordinate with the Alfred Health HITH unit and external providers to ensure seamless care.

The role requires current RN registration with NMBA via AHPRA, solid interpersonal and organisational skills, and a valid Victorian driver's licence.
