We're seeking a compassionate and proactiveHome Care Coordinatorto join our clients dedicated team and help ensure older adults and individuals with support needs can live independently and safely at home.About the RoleAs a Home Care Coordinator, you'll play a pivotal role in planning, coordinating, and monitoring care and support services for clients.
Working closely with clients, families, and support staff, you'll ensure services are delivered with compassion, professionalism and efficiency.Your day-to-day will include:Managing incoming referrals and conducting client assessmentsDeveloping individualised care plans tailored to client goals and needsCoordinating staff schedules and matching carers with clientsMonitoring service delivery to maintain quality and client satisfactionMaintaining accurate client records and complying with relevant policies and proceduresResponding promptly to incidents, feedback,
