Job Description

Uniting Care Queensland is seeking a Supported Living Facilitator on a casual contract basis in Mount Gravatt. You will work with the person supported, family and networks to support daily living with choice and quality of life.

You will contribute to developing, implementing and reviewing individual plans to promote autonomy and dignity, while upholding privacy and confidentiality and ensuring respectful care within the FaDS team across Queensland.
