For Purpose Aged Care Australia in South Bunbury is seeking a compassionate Social Worker for a part time, 12-month contract.
You will provide psychosocial support, advocacy, and case management to residents and families, especially during hospital-to-care transitions.
Requirements: Bachelor of Social Work (or equivalent) and eligibility for AASW, with experience in health or aged care.
