Job Description
APM is seeking a Clinical Aged Care Assessor to identify needs of older Australians and connect them with services to support independent living.
The role is perm part time (4 days per week) with daily travel across the service area.
The successful candidate will conduct high-quality assessments, facilitate service referrals, promote functional daily living, and engage with families and carers while complying with Health Department requirements.
