CN - Gladstone Hospital - Adult Community (Case Management)
Gladstone Hospital's Adult Community Mental Health team is looking for a Clinical Nurse to join their case management service in Gladstone, Queensland.
This is a contract position classified at NRG 6, starting as soon as possible and running through to 18 October ****.
You'll be working in community-based mental health, supporting adults through coordinated care and case management in a role that makes a real difference to people's lives.
Community-based case management in adult mental health, working directly with clients and their support networks
Contract position, NRG 6 classification, versatile start date to suit your availability
Based in Gladstone with the opportunity to develop expertise in community mental health nursing
Preferred Requirements
Solid experience in adult mental health nursing,



with a focus on community-based care
Demonstrated knowledge of case management principles and coordinated care delivery
Robust communication and relationship-building skills, particularly with vulnerable populations
Ability to work autonomously and manage a caseload effectively
Current knowledge of mental health legislation, policy, and best practice in Queensland
Preferred Qualifications
Registered Nurse qualification with current AHPRA registration
Post-graduate qualification or demonstrated experience in mental health nursing
Case management certification or equivalent professional development in community mental health
