Job Description
Barwon Health in Geelong is seeking a Carer Support Clinician within Community Health.
The role focuses on government funded carer support services, delivering information, guided referrals, peer support, outreach and practical support to carers.
You will work in a multidisciplinary setting, undertaking assessments, coordinating services and helping carers build capacity and resilience through coaching and support.
