Job Description
Bayside Health is recruiting a social worker for the At Home Subacute service based at Peninsula University Hospital in Frankston. The role involves delivering social work assessments, counselling and discharge planning across home visits, clinics, group programs and telehealth.

You will work in a energetic, 7-day service with ongoing supervision, professional development and collaboration with allied health, nursing, pharmacy and medical teams.
