At Home Together is seeking an Aged Care Coordinator / Care Partner in Canberra to manage a portfolio of clients and oversee care journeys from onboarding through ongoing service coordination.
Preference for ENs/RNs with community aged care experience.
You will work with clients, families and healthcare professionals to ensure secure, person-centred care aligned with individual plans and budgets.
Office-based in Phillip, ACT with travel locally.
