Summary:

The Care Coordinator will be responsible for coordinating patient care services, advocating for patient needs, and ensuring effective communication between healthcare professionals, patients, and their families or caregivers.

Responsibilities:

Coordinate and manage patient care services based on individual needs and resources
n
Conduct comprehensive assessments and develop individual care plans
n
Collaborate with healthcare professionals and other care coordinators
n
Advocate for patients' access to necessary medical services and resources
n
Schedule and monitor appointments, referrals, and care transitions
n
Communicate with patients and families to provide guidance and education on disease management and self-care strategies
n
Maintain accurate patient records and care plans
n




Participate in team meetings and quality improvement initiatives
n

Qualifications:
n
n
Bachelor's degree in Healthcare Administration, Nursing, Social Work or related field
n
Minimum of two years of experience in healthcare or care coordination
n
Knowledge of healthcare delivery systems and care coordination techniques
n
Excellent organizational, communication, and interpersonal skills
n
Ability to work independently or as part of a team in a fast-paced setting
n
Strong analytical and problem-solving skills
n
Commitment to patient-centered care and empathy for patient needs
n
Proficiency in medical terminology, electronic health records, and Microsoft Office Suite
n
