You will conduct intake, develop personalised care plans and support clients in daily living, transport and social activities.
Role requires Certificate III Aged Care or equivalent, a current Australian driver's licence and a roadworthy vehicle for work.
You will work on the road, building meaningful relationships with clients in their homes.
