Your Home. Your Care. Your Opportunity.

Come and make the Poppy's Difference!

Two Full-Time Care Partner Positions | Registered Nurses Encouraged to Apply

We're growing — and we'd love you to grow with us!

At Poppy's Home Care, we're doing things a little differently.

We're a local, family-owned and nurse-led home care provider, proudly supporting older Australians across our region.

We're incredibly proud of what we're building. More and more clients and families are choosing Poppy's because they value genuine relationships, personal service and a team that truly cares.

And the exciting part? We're only just getting started!

With a brand-new office in Port Kennedy under construction and exciting plans for the future, we're looking for two exceptional Care Partners who share our passion and want to be part of the journey.

A great team deserves great support!

We believe we have one of the best home care teams around, and we're incredibly proud of the people who make Poppy's what it is.

Our team goes above and beyond for our clients every day. In return, we believe they deserve the support, encouragement and resources to do their jobs well.

We look after our team, so they can look after our clients.

Why you'll love working at Poppy's

- Manageable caseloads — time to get to know your clients, build meaningful relationships and deliver great care.

- A genuinely supportive team — experienced clinical and operational guidance, friendly colleagues and people who have your back.

- Approachable, hands-on owners — your voice matters, your ideas are welcomed and your contribution is valued.

- Happy clients and families — people who appreciate the difference personalised care makes.

- A brand-new office on the way — an exciting new home for our growing Port Kennedy team.

- A business with a bright future — an opportunity to grow professionally and help shape what comes next.

The prospect

As a Care Partner, you'll play an important role in helping older Australians remain independent, safe and supported in their own homes.

This is a professional, predominantly office-based case management and care coordination role, with client visits as required. It is not a hands-on support worker position.

You'll take ownership of your own client caseload, with responsibilities including:

- Building trusted relationships with clients and their families.

- Developing and reviewing personalised care plans.

- Coordinating home care services, allied health and external providers.

- Managing client budgets, funding and expenditure.

- Supporting clients to understand their Support at Home services and contributions.

- Responding to changing needs, concerns and incidents.

- Maintaining accurate records and ensuring actions are followed through.

Could you be our next Care Partner?

We're looking for someone who combines a genuine passion for aged care with the professional skills and confidence to manage a busy, rewarding caseload.

What matters most to us:

- Proven aged-care case management, care coordination or comparable professional experience.

- Excellent IT skills and confidence working across multiple systems.

- Strong administrative, organisational and time management skills.

- Confidence with budgets, figures and financial information.

- Excellent communication, initiative and professional judgement.

- A caring, positive attitude and genuine commitment to older Australians.

- Availability to work full-time.

Registered Nurses are particularly encouraged to apply, as we're looking to further strengthen the clinical expertise within our team.

We also welcome applications from exceptional Enrolled Nurses and experienced non-nurse aged-care case managers who meet the role requirements.

Knowledge of the Support at Home program, including budgets, funding and client contributions, is highly desirable. If you're new to Support at Home but bring strong case management experience and the other essential skills, we'd still love to hear from you.

What we offer A full-time position with a growing, family-owned business where you'll be supported, appreciated and given the opportunity to make a genuine difference.

Salary negotiated according to qualifications, relevant experience and responsibilities, plus a $10,000 annual car allowance.

Ready to make the Poppy's Difference?

We're not just looking to fill two positions. We're looking for two wonderful people who will share our values, take pride in their work and become an significant part of our Poppy's family.

If you're looking for somewhere you can feel valued, do meaningful work and be part of something exciting, we'd love to hear from you!

Apply through SEEK and tell us a little about yourself, your experience and why you'd love to join Poppy's.
