About the Role

Reporting to the Head of Aged Care, the Aged Care Lead provides senior operational leadership across our Aged Care programs, including Support at Home and CHSP services.

You'll be responsible for translating strategic priorities, aged care reform requirements and care governance expectations into sustainable, high-quality service delivery outcomes.

Leading across service delivery, workforce capability, quality, risk, financial performance and continuous improvement, you'll work closely with leaders across Care Management, Clinical, Scheduling, Customer Service and other key functions to ensure operational excellence and a consistent client experience.

A key focus of the role will be supporting Care Connect's continued growth under Support at Home while maintaining service quality, workforce readiness and long-term sustainability.

Key Responsibilities

- Lead operational performance across Aged Care programs.
- Drive safe, effective and person-centred service delivery.
- Lead workforce planning,



resource management and operational sustainability.
- Monitor and improve service, quality, workforce and financial performance.
- Provide leadership and oversight of governance, risk and compliance activities.
- Support implementation of aged care reforms and new service models.
- Lead continuous improvement and operational transformation initiatives.
- Build capability, accountability and performance across leaders and teams.
- Develop strong internal and external stakeholder relationships.
- Contribute to organisational strategy, growth and service innovation.

About You You are a values-driven and commercially aware leader who thrives in complex environments and enjoys balancing client outcomes, quality expectations and operational performance.

You bring a deep understanding of aged care service delivery and a passion for supporting teams to deliver safe, sustainable and person-centred care.

To be successful, you will have:

- Demonstrated senior leadership experience within aged care, home care, health, disability or community services.
- Relevant qualifications in aged care, health, community services, business, management or a related discipline.
- Experience leading services through periods of reform and organisational change.
- Knowledge of Support at Home, CHSP and contemporary aged care service delivery models.
- Experience leading leaders and multidisciplinary teams.
- Strong understanding of aged care reform, quality standards, care governance and risk management.
- Experience managing operational performance, workforce planning, budgets and service sustainability.
- A proven track record leading change, transformation and continuous improvement initiatives.
- Robust stakeholder engagement,



communication and influencing skills.
- Experience using data and insights to drive operational decision-making.
- A collaborative and authentic leadership style aligned with Care Connect's CARE values.
