Your role
As a Care Coordinator, you’ll help make sure our participants receive consistent, personalised support and our support workers have what they need to deliver it well.
Working closely with senior management and our clinical team, you’ll coordinate services, manage rosters and build trusted relationships with participants, families and staff. You’ll stay across the details, anticipate issues and keep things moving when plans change.

What you’ll be doing
- Building strong relationships with participants, families,
support workers and other professionals involved in their care.
- Conducting home visits and care assessments within your scope, and working with our clinical team to develop and review personalised care plans.
- Coordinating services for participants with a range of needs, including complex circumstances and medical requirements.
- Managing rosters, staff availability, leave and shift changes, matching the right people to each participant.
- Keeping participants and staff informed about their services, rosters and any changes.
- Supporting and guiding support workers, monitoring service quality and following through on concerns.
- Responding to enquiries and coordinating care consultations, appointments and in-home services.
- Assisting with recruitment, onboarding and staff compliance.
- Maintaining accurate care records, risk assessments, emergency plans and other essential documentation.
- Reviewing rosters for compliance and efficiency, and identifying ways to improve how we work.
Who you are You’re warm, capable and easy to connect with. You bring positive energy, communicate openly and follow through on what you say you’ll do.
You understand that a roster change can have a big impact on someone’s day. You’re comfortable having difficult conversations,
know when to ask for support and can work through competing priorities without losing sight of the person receiving care.
You’ll bring
- At least two years’ experience in care coordination or support coordination, preferably in disability or aged care, with a sound understanding of service delivery and rostering.
- A tertiary qualification in Disability, Human Services, Social Work, Mental Health or a related field.
- Strong organisational skills and the ability to manage a busy, changing workload.
- Clear, thoughtful communication and confidence building relationships with people from different backgrounds.
- Initiative, sound judgement and a practical approach to solving problems.
- A collaborative attitude and a willingness to support your teammates.
Experience in a clinical or nursing role, exposure to complex care, and knowledge of the NDIS will be highly regarded. Why join LunaCare?
