Case Manager – Brain and Spinal Cord Injury

About us
Crescent Care Foundation is a Liverpool-based NDIS-registered provider and ACNC-registered charity. We support people with disability and their families across South Western Sydney, Western Sydney and the Blue Mountains, with a solid focus on culturally and linguistically diverse communities.
We are applying to join icare’s Approved Case Managers (Lifetime Care) Prequalification Scheme (SCM11991). We are looking for experienced case managers to nominate with our application and to work with people who have severe injuries from motor vehicle or workplace accidents.
The role
You will provide case management for people with traumatic brain injury, spinal cord injury and other severe injuries, across the rehabilitation journey from hospital discharge to living in the community. This includes:
- person-centred planning and goal setting with participants and families
- coordinating rehabilitation, allied health, attendant care and community services
- assessing risk and putting safeguards in place
- preparing plans and service requests under the Lifetime Care Guidelines and workers compensation requirements
- monitoring outcomes and working closely with icare
Essential requirements (set by icare, so we cannot waive them)
- A Bachelor’s degree or higher in Occupational Therapy, Social Work, Nursing, Psychology, Physiotherapy, Speech Pathology, Exercise Physiology, Rehabilitation Counselling, or another health, rehabilitation, disability or human services discipline accepted by icare
- At least 5 years of full-time (or equivalent) professional experience in person-centred case management for people with severe injuries, disabilities and complex needs. Student placements and practicums do not count.
- Availability for at least 20 hours a week across 4 days during business hours
- Current Ahpra registration, if your profession requires it
- A National Police Check, and a Working With Children Check if you will work with people under 18
Highly regarded
- A second language is highly regarded, such as Arabic, or being bilingual in another community language
- Experience with children and young people, people ageing with a severe injury, or people with challenging behaviours after brain injury
- Experience with Lifetime Care, Workers Care, CTP or other compensation schemes
- Current icare approval as a case manager. Everyone currently approved must re-apply in this intake, and we welcome applications from approved case managers.
- Living in or near South Western or Western Sydney
Engagement options
- Contract or subcontract (ABN): 75% of the hourly rate icare approves for each referral, paid for approved hours.
- Start date: when icare approves our application, expected early 2027
- Referrals come from icare and depend on matching participants to case managers.

icare requires each case manager to complete their own form. We will give you the form and the criteria, but we cannot write it for you.

Pay: $90,000.00 – $130,000.00 per year
