Your role
As our Clinical Care Coordinator, you’ll help manage our Supported Independent Living (SIL) homes and participants, bringing a nursing perspective to the everyday care of our participants. You’ll work closely with care coordinators, support workers and our existing clinical team to make sure supports are well organised, responsive and suited to each person’s needs.
You’ll stay connected to what’s happening in our homes, get to know participants and their support teams, and provide practical guidance on their health and care needs. From recognising changes in someone’s condition to reviewing care plans and helping staff respond confidently, your nursing experience will inform how we deliver care each day.
This is a hands-on role with variety, shared support and room to grow. You’ll also assist with care coordination and day-to-day service delivery, helping keep our homes running smoothly and our teams supported.
What you’ll be doing
- Leading clinical assessments and developing, implementing and reviewing individualised care plans that reflect each participant’s needs, preferences and goals.
- Providing clinical oversight for participants with complex care needs, recognising changes in their condition and ensuring timely intervention and escalation.
- Identifying and managing clinical risks, helping prevent avoidable complications and improving participant outcomes in community settings.
- Providing clear clinical direction to care coordinators and support workers, supporting informed decisions and consistent care.
- Delivering practical education, mentoring and guidance to build staff confidence and capability in supporting complex needs.
- Working with participants, families, treating practitioners, hospitals, allied health professionals and other stakeholders to coordinate care and support transitions between services.
- Contributing to multidisciplinary discussions and advocating for care that respects each participant’s choice, dignity and independence.
- Maintaining accurate clinical documentation and contributing to incident reviews, clinical policies, quality improvement and clinical governance.
- Supporting operational priorities when needed, including care coordination, rostering and service delivery, to maintain safe and reliable supports.
- Helping strengthen our clinical systems and practices as LunaCare continues to grow.
Who you are You’re an approachable, practical RN who enjoys building relationships and being involved in the everyday delivery of care. You notice when something needs attention, take initiative and work with others to find a solution.
You’re comfortable guiding support workers, talking things through with families and drawing on your colleagues’ expertise when needed. You understand that managing a home well means paying attention to both people’s health needs and the everyday details that make it a comfortable, supportive place to live.
You bring warmth, positive energy and a willingness to get involved. You’re keen to contribute your nursing knowledge, develop your leadership skills and grow with a team that looks out for one another.
