Role Description The Community Care Manager is a full-time, on-site role based in Ashfield, NSW, responsible for overseeing the coordination and delivery of care and support services to residents. Day-to-day tasks include managing care plans, supervising care staff, and ensuring that residents receive appropriate clinical, social, and emotional support. The role involves liaising with families, healthcare providers, and community services to maintain continuity and quality of care.
The Community Care
Manager will monitor compliance with relevant standards and policies, support staff training and development, and contribute to continuous improvement initiatives within the facility.
Qualifications
- Demonstrated skills in case management, including assessment, care planning, and ongoing review.
- Experience in elder care and residential aged care environments, with a strong understanding of person-centered support.

- Relevant knowledge of medicine or clinical care in an aged care context, working collaboratively with healthcare professionals.
- Formal qualifications in nursing, social work, aged care, or a related discipline.
- Strong communication, leadership, and team management abilities.
