Role Overview

We are looking for a passionate and reliable Disability Support Worker to join our team in Kilkenny, SA. In this rewarding role, you will work closely with participants to implement individual support plans, promote functional independence, and facilitate meaningful engagement in everyday life activities.

Key Tasks and Duties

- Individual Plan Implementation: Actively apply personalized care strategies tailored to each participant's individual goals, abilities, and unique lifestyle choices.
- Daily Living & Routine Facilitation: Assist participants in developing and maintaining essential household management skills, such as meal planning, light domestic organization, and personal hygiene routines.
- Community Access & Mobility Support: Accompaniment and transport for participants to social outings, medical appointments, educational courses, and recreational groups to foster social connection.
- Skill Building & Empowerment: Guide participants in learning self-advocacy, decision-making,



and budget management to enhance their long-term personal independence.
- Observation & Progress Reporting: Document participant milestones, daily progress, and any subtle physical or behavioral shifts, escalating concerns to care managers or medical professionals when appropriate.
- Health & Routine Assistance: Provide support with daily health maintenance, including prompting scheduled medications, encouraging physical activities, and ensuring protected mobility techniques.
- Safety & Environment Monitoring: Maintain a safe, respectful, and dignified physical environment in line with relevant workplace health and safety protocols and organizational policies.

Requirements & Qualifications

- Certificate III or IV in Individual Support, Disability, or equivalent relevant qualification or 2 Years Work experience will substitute for the formal qualification
- Relevant experience working with individuals with diverse abilities.
- Current NDIS Worker Screening Check and Working with Children Check (WWCC).
- Valid First Aid and CPR Certification.
- Current Australian Driver’s License and reliable transport.
- Strong interpersonal skills with an empathetic and proactive approach.
