Tarneit

About the company

Care Fast Group is a registered NDIS provider specialising in Supported Independent Living and support for people with complex needs. We work with participants, families and health professionals to provide personalised support that promotes dignity, choice and community inclusion.

We are building a dedicated team to provide SIL support in a High Physical Support SDA home in Tarneit.

About the role

We are seeking experienced Disability Support Workers with practical skills in PEG feeding, complex personal care and manual handling to join a team providing 24-hour support.

Support responsibilities:

- Provide full assistance with showering, dressing, continence care, oral hygiene and daily routines.
- Complete two-person hoist transfers, assisted rolling and bed repositioning according to the manual-handling plan. Use prescribed slings, slide sheets and positioning equipment safely.
- Support safe positioning in bed and in a tilt-in-space wheelchair, follow pressure-care routines and report skin concerns.
- Provide PEG feeding, prescribed flushes and PEG-site care after participant-specific training and competency assessment, following the enteral nutrition plan.
- Assist with giving prescribed medicines, including through a PEG feeding tube, once trained, assessed as competent and authorised to do so.
- Complete blood glucose checks and follow the documented recording and escalation instructions.
- Assist with oral meals and drinks according to prescribed mealtime and positioning instructions.
- Follow the communication plan, explain care before touching or moving the participant, and provide reassurance throughout transfers.
- Observe comfort, breathing,

posture and general wellbeing. Promptly report non-verbal signs of pain, distress or changes in health.
- Support community access, appointments, leisure activities and daily household routines. Maintain accurate care records and clear shift handovers.

About the participant You will support a participant with an acquired brain injury who requires comprehensive assistance with daily living and ongoing monitoring of health and comfort. Their care includes PEG feeding, medicines through a PEG feeding tube, blood glucose monitoring, pressure care and complex mobility support. Limited verbal communication makes careful observation and recognition of subtle changes particularly important.

Essential requirements

- Demonstrated experience supporting people with complex physical disabilities and providing full assistance with personal care.
- Practical PEG feeding experience and relevant enteral-support training.
- Experience with two-person hoist transfers, manual handling, pressure care and repositioning.
- Experience supporting people with limited verbal communication or complex communication needs.
- Current NDIS Worker Screening clearance, First Aid and CPR certification.
- Completion of the NDIS Worker Orientation Module before commencement.

- Valid Australian work rights and ability to reliably travel to Tarneit.
- Willingness to complete participant-specific training and practical assessments before independently performing specialised tasks.
- Certificate of completion in hand hygiene.
- Current annual Influenza vaccination is required (or valid certified medical exemption). COVID-19 vaccination is optional but desirable.
- Knowledge of complex bowel care and the ability to follow an individual bowel-care plan after appropriate training.

Available shifts and training Care Rapid will provide support 24 hours a day, seven days a week. Please provide your availability for morning, afternoon, evening, overnight and weekend shifts. Exact shift times and overnight arrangements will be confirmed. You may also be offered shifts with other participants across the region.

Successful applicants will complete induction, participant-specific training and competency assessment. Two staff will work together for all tasks requiring two-person assistance under the care and manual-handling plans.
