About the company
Care Quick Group is a registered NDIS provider specialising in Supported Independent Living and support for people with complex needs. We work with participants, families and health professionals to provide personalised support that promotes dignity, choice and community inclusion.
We are building a dedicated team to provide SIL support in a High Physical Support SDA home in Tarneit.
About the role
We are seeking Registered Nurses to provide scheduled short nursing shifts and work closely with disability support workers and treating health professionals to deliver consistent, safe care.
Nursing responsibilities
- Assess the participant’s health during nursing shifts, including comfort, pain, breathing, hydration, nutrition and skin condition. Recognise and act on changes from their usual presentation.
- Provide PEG feeding, prescribed flushes and PEG-site assessment and care in accordance with the current clinical plan.
- Administer prescribed medicines, including through a PEG feeding tube, and maintain accurate medication records. Identify and escalate medication concerns.
- Complete blood glucose checks, document results and respond according to the prescribed monitoring and escalation plan.
- Assess skin integrity and pressure-care needs, provide wound management where clinically required and within your competence, and arrange further clinical review when needed.
- Follow the communication plan and use observation to assess non-verbal signs of pain, discomfort and distress.
- Contribute to nursing care plans and clear escalation instructions in consultation with the treating team and Care Fast’s clinical lead.
- Guide support workers in safe care and contribute to participant-specific training and competency assessment within your expertise. Undertake delegation and supervision in accordance with professional standards and organisational arrangements.
- Follow the manual-handling plan and work with a second trained staff member for tasks requiring two-person assistance.
- Maintain clinical notes and clear handovers. Coordinate with treating practitioners, allied health professionals and the service-delivery team.
- Complete prescribed blood glucose monitoring. Where included in the clinical plan, support continuous glucose monitoring (CGM), respond to sensor alerts, change sensors and administer prescribed insulin within your competence and the agreed diabetes-management arrangements.
About the participant You will support a participant with an acquired brain injury who requires comprehensive assistance with daily living and ongoing monitoring of health and comfort. Their care includes PEG feeding, medicines through a PEG feeding tube, blood glucose monitoring, pressure care and complex mobility support. Limited verbal communication makes careful observation and recognition of subtle changes particularly important.
Essential requirements
- Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia through Ahpra.
- Recent hands-on nursing experience relevant to complex disability, neurological, rehabilitation or community care.
- Demonstrated competence in PEG feeding, PEG-site care and medication administration through a PEG feeding tube.
- Experience with blood glucose monitoring, skin assessment, pressure-injury prevention and recognition of clinical deterioration.
- Ability to assess people with limited verbal communication and follow individual communication strategies.
- Clear documentation, effective handover and ability to guide support workers within agreed clinical responsibilities.
- Current NDIS Worker Screening clearance and First Aid and CPR certification.
- Valid Australian work rights and reliable travel to Tarneit for short shifts.
- Willingness to complete participant-specific induction and training before providing care.
- Current annual Influenza vaccination is required (or valid certified medical exemption). COVID-19 vaccination is optional but desirable.
Available shifts and training Scheduled short nursing shifts will support the participant’s clinical care within the 24-hour service. Exact shift lengths, visit frequency and times will be confirmed according to the final clinical support arrangements.
Registered
Nurses may also work in a Support Worker role, with additional support-worker hours offered before or after nursing shifts, subject to participant needs, suitability and roster availability. The duties, pay rate and arrangements for each role will be agreed before rostering.
Please state your availability for short visits, weekends and any early morning or evening work. Successful applicants will complete participant-specific induction and review the relevant clinical, communication and manual-handling plans.
