Experienced Support Coordinator

Part-time / Permanent

Dream Pursue Grow Support is a fast-growing, NDIS registered, values driven organisation committed to helping people build confidence, exercise choice and pursue meaningful goals. We are seeking an experienced Support Coordinator who can confidently manage a participant caseload, navigate the NDIS and build strong relationships with participants, families, providers and community services. This role is ideal for a capable and organised professional who combines sound NDIS knowledge with a caring, patient, practical and person-centred approach.

About the Role

As a Support Coordinator, you will work alongside participants to understand and implement their NDIS plans, connect with suitable supports / provider and assist them about making informed decisions about their services. You will help participants strengthen their capacity to coordinate supports, overcome barriers and work towards greater independence. You will also monitor service delivery, identify emerging risks and support participants through plan reassessments and changes in circumstances.

Key Responsibilities

- Manage and grow a caseload while providing responsive, participant-focused service.
- Help participants understand and implement their NDIS plans.
- Support participants to identify goals, priorities and appropriate services.
- Research, connect and coordinate mainstream, community, informal and funded supports.
- Build participants' confidence and capacity to exercise choice and control.
- Develop effective working relationships with participants, families, nominees, providers and allied health professionals.
- Monitor plan budgets, service delivery and progress towards participant goals.




- Assist participants to prepare for plan reassessments, reviews and changes in circumstances.
- Maintain accurate, timely and professional case notes, reports and other documentation.
- Work in accordance with NDIS Practice Standards, relevant legislation and organisational policies.
- Contribute ideas and experience as the organisation develops and grows.

About You You are an experienced Support Coordinator who can balance empathy with accountability. You build trust while maintaining clear skilled boundaries, and you are comfortable managing competing priorities in a changing workplace.

You will bring:

- Demonstrated experience in NDIS Support Coordination or a closely related role.
- Strong working knowledge of the NDIS, participant plans and funded support categories.
- Experience coordinating services for participants with diverse or complex needs.
- Strong assessment, problem-solving and risk-management capabilities.
- Excellent written communication, reporting and case-note skills.
- Confidence communicating with participants, families, providers and other stakeholders.
- The ability to manage a caseload independently and meet agreed timeframes.
- Sound judgement, initiative and attention to detail.
- A commitment to participant rights, dignity, autonomy and informed decision-making.
- A collaborative and adaptable approach suited to a fast-growing organisation.

Essential Requirements

- A relevant qualification in community services, disability, social work, psychology, mental health or a related discipline.




- Previous experience in NDIS Support Coordination.
- Current NDIS Worker Screening Check.
- Current Working with Children Check, where required.
- Current First Aid and CPR certification.
- Australian driver's licence.
- Access to a reliable and insured vehicle.
- Ability to satisfy all relevant NDIS worker compliance requirements.
- Right to work in Australia.
